Schedule 1 Facilities Class Action

The deadline to file a claim has been extended and is now April 28, 2017.


Under the settlement, class members had until February 28, 2017, to apply for compensation.  This deadline has been extend to April 28, 2017 to allow class members more time to make claims.


If you do not submit a claim by April 28, 2017, you will not be able to receive any compensation.

If you want to ask for money from the Settlement you must fill in a separate Claim Form for each of the facilities you lived at and send it to the Administrator by April 28, 2017 by mail, fax or email at:

Please note that if you are mailing your claim it must be postmarked by Canada Post on or before April 28, 2017.  If you are faxing or emailing your claim it must be received by 11:59PM (EST) on April 28, 2017.


If you lived at more than one facility you must submit a separate claim form for each facility you attended.


Claim form packages were mailed during the week of May 30 – June 3, 2016 to those persons who previously signed up to receive a claim form.  If you are not sure if you asked to receive a claim form, please call 1-866-442-4465 (TTY: 1-877-627-7027).

It will take quite a while for the claims office to review all of the claims. It may be months before you hear anything from the claims office. Please try to be patient. The claims office will contact you in writing if they have any questions or once they have made a decision about your claim.
